Medical/Pharmacy Benefits

Dental Insurance Plan
- Those eligible: full-time employees
- Dental coverage for employee and eligible dependents
- Orthodontic coverage for children under the age of 19
Flexible Spending Accounts (FSA)
- Those eligible: full-time and part-time employees
- Allow medical and dental premiums to be deducted pretax
- Medical Spending Account – Ability to pay out-of-pocket health/dental expenses (expenses must be eligible under IRS Section 125 guidelines) that are not covered by medical, dental or vision plans with pretax dollars
- Daycare Spending Account – Ability to pay IRS Section 125 eligible daycare expenses that are incurred while working with pretax dollars
- Election amounts for each of the accounts are based on IRS limits per calendar year
Basic Life Insurance
- Those eligible: full-time employees
- SFHS will provide a base annual earnings amount in basic life insurance and accidental death/dismemberment (AD&D) (up to a maximum benefit limit)
Optional Term Life Insurance
- Those eligible: full-time employees
- Employees may purchase an additional one, two, three, four, five or six times base annual earnings life insurance (up to maximum benefit limit)
- May be subject to underwriting
Dependent Life Insurance
- Those eligible: full-time employees
- Employees may purchase dependent life insurance for eligible dependents
- May be subject to underwriting
Voluntary AD&D
- Those eligible: full-time employees
- Employees may purchase additional AD&D, from one to ten times salary (up to maximum benefit limit )
- Employee only or family coverage
Long Term Disability Insurance
- Those eligible: full-time employees
- SFHS will provide insurance to eligible employees based on consecutive employment and exempt and non-exempt status
- McAlester employees are excluded
Critical illness Insurance- Those eligible: full-time employees
- Guaranteed issued policy for employee and eligible dependents
- Must be enrolled in a medical plan to qualify for coverage
Basic Retirement Plan
- Those eligible: full-time, part-time, and ECB employees
- Eligible after 12 months of employment, paid 1,000 hours, and age 21
- SFHS will contribute a designated percentage of salary to a retirement account for employees
401k Plan
- Those eligible: full-time, part-time and ECB employees
- Immediate upon hire; automatically enrolled in the plan
- Employees may opt in/opt out/increase/decrease their contributions at any time
- After 12 months of employment, paid 1,000 paid hours and at least 21 years of age, SFHS will match a designated percentage of employees contributions
Paid Time Off (PTO)
- Those eligible: full-time and part-time employees
- Start accruing immediately upon employment
- Paid time off for vacations, personal use, holidays and the first 24 consecutive hours of personal illness or injury
- May carry over unused PTO hours to PTO bank (up to maximum PTO bank limit).
- Full-time and part-time employees accrue PTO hours based on hours worked and length of service
Saint Francis Health System recognized Holidays
- New Year’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day
- Part-time and ECB employees receive an extra hour of base pay up to a maximum of 8 hours worked on a holiday
Extended Sick Leave (ESL)
- Those eligible: full-time employees
- Start accruing immediately upon employment
- Paid time off for personal illness or injury lasting more than 24 consecutive scheduled hours
- May accrue up to a certain number of days per year and carry over unused hours (up to maximum ESL bank limit)
Family and Medical Leave of Absence (FMLA)
- Those eligible: full-time, part-time and ECB employees
- Eligible after 12 months of employment and 1,250 hours worked prior to leave of absence
- Up to 12 weeks leave with retention of benefits and job following a temporary absence required due to birth or placement of a child or serious medical condition of employee, spouse, child or parent (in accordance with policy)
- Leaves are subject to approval by absence management company based on DOL guidelines
Military Leave of Absence (LOA)
- All employees who are engaged in the uniform services
- Leave approval in accordance with USERRA Guidelines
Medical Leave of Absence (MLOA)
- Those eligible: full-time and part-time employees not eligible for FMLA or have exhausted their FMLA
- Leaves are subject to approval by absence management company
Tuition Assistance Plan
- Those eligible: full-time and part-time employees
- After completion of 6 months of continuous employment in an eligible status.
- May receive reimbursement after class completion and necessary documentation is provided based on annual limits per policy guidelines
- Requires employee to remain in a full-time or part-time working status for six months following completion of the course
- Critically needed degrees may be eligible for a larger reimbursement amount
Health Zone
- Those eligible: full-time, part-time and ECB employees
- Immediate upon employment
- Employee discount for Health Zone membership
Ave Maria House
- Those eligible: full-time, part-time and ECB employees
- Immediate upon employment
- The Child Development Center is open Monday through Friday and is located on the Saint Francis Hospital campus
- The School Age Program for the Four Year Old program and the Before/After school program operates Monday through Friday and is located at 5710 E. 61st Street South
Service Awards
- Those eligible: full-time, part-time and ECB employees
- Five years of continuous employment required
- Service award gift and special recognition honoring employee's anniversary beginning on the 5th year and continuing in five-year increments
Jury Duty Policy
- Those eligible: full-time and part-time Immediate upon employment
- Time paid for jury duty is not deducted from PTO
Bereavement Leave/Funeral Pay
- Those eligible: full-time employees
- Immediate upon employment
- Up to three days leave/24 hours of funeral pay for policy defined family members
Adoption Benefits
- Those eligible: full-time and part-time employees, one year of continuous service from most recent hire date
- Adoption assistance amount is per adoption and based on employee’s FT or PT status
- If employee and spouse are both employed by SFHS, eligible amounts are allowed to one employee per family
Credit Union

- A full service financial institution providing savings, investment and lending services
