Saint Francis Hospice Team

Saint Francis Hospice utilizes a multidisciplinary approach to the management of patients. This means many different medical specialties come together to work toward the goal of controlling the patient's pain and symptoms. The Saint Francis Hospice team consists of:

  • Personal Physician: Coordinates the complete medical care of the hospice patient
  • Medical Director: Provides medical direction for the hospice program and acts as a resource for the hospice team
  • Registered nurses: Specially-trained nurses provide expertise in pain and symptom management; RNs make home visits and assist with all aspects of the patient's care.
  • Home Health Aides: Make home visits to assist with activities of everyday living, such as bathing and dressing
  • Social Workers: Assist the patient and family with legal and financial services
  • Spiritual Counselor: Non-denominational counseling available for affirmation of faith or assistance in finding spiritual peace
  • Pharmacists: Assist with medication management
  • After-Hours Case Managers: Specially-trained registered nurses who are available and on-call seven days a week, 24 hours a day
  • Volunteers: Trained individuals available to be a "good neighbor" to the patient and family by providing a variety of good deeds
  • Bereavement Support Counseling: Provides bereavement services to families or significant others of hospice patients for 13 months after the death of the patient through mail or phone correspondence; Individual counseling and support groups are also available.
