  Medical/Pharmacy Benefits
  • Those eligible: full-time and part-time employees
  • Comprehensive plans for employees and eligible dependents.
  • You can choose between the Saint Francis Health System PPO, HDHP or CommunityCare HMO.
Dental Insurance Plan
  • Those eligible: full-time employees
  • Dental coverage for employee and eligible dependents. Orthodontic coverage for children under the age of 19.
TaxSaver Plan
  • Those eligible: full-time and part-time employees
  • Allow medical and dental premiums to be deducted Pre-Tax
  • Medical Spending Account – Ability to pay out-of-pocket health/dental expenses (expenses must be eligible under section 125 guidelines) that are not covered by medical, dental or vision plans with pre-tax dollars.
  • Daycare Spending Account – Ability to pay section 125 eligible daycare expenses that are incurred while working with pre-taxed dollars.
  • Election amounts for each of the accounts are a minimum of $260 and a maximum of $5,000 per year.
  • Deductions are withheld in equal installments throughout the year.
  • Reimbursements received are non-taxable.
  • This is a use-it-or-lose it program.
  • Contribution amount must be re-elected each year during Open Enrollment for the following calendar year.

Life Insurance

  • Those eligible: full-time employees
  • SFHS will provide 1x base annual earnings in basic life insurance and accidental death/dismemberment (AD&D).
  • Maximum benefit of $250,000
Optional Term Life Insurance
  • Those eligible: full-time employees
  • Employees may purchase an additional one, two, three, four, five or six times base annual earnings life insurance.
  • May be subject to underwriting
  • Maximum benefit of $1.5 million
Dependent Life Insurance
  • Those eligible: full-time employees
  • Employees may purchase dependent life insurance equal to $10,000 for spouse/ $5,000 for children over 6 months and $200 for children under 6 months.
  • May be subject to underwriting

Voluntary AD&D

  • Those eligible: full-time employees
  • Employees may purchase additional AD&D, from one to ten times salary, maximum benefit $500,000
  • Employee only or family coverage

Long Term Disability Insurance

  • Those eligible: Exempt - upon full-time employment, Hourly - upon first of month following two consecutive years of full-time employment, McAlester employees excluded.
  • 60% of base monthly earnings (not to exceed $10,000) payable to age 65 if employee continues to meet disabled definition according to plan.
  • Elimination period (period of disability before benefits start ): Exempt - 90 days, Hourly - 180 days.

Basic Retirement Plan

  • Those eligible: full-time, part-time, and ECB employees
  • Eligible after 12 months of employment, paid 1,000 hours, and age 21
  • SFHS matches 6% of salary and contributes it to a retirement account

401k Plan

  • Those eligible: full-time, part-time and ECB employees
  • Individual pre-tax retirement savings
  • Immediate upon hire
  • Employees are AUTOMATICALLY enrolled in the plan at 3% after your 30 day election period has ended
  • Employees have 30 days from hire date to opt out by calling Fidelity at 1(800)343-0860 or logging onto www.Fidelity.com/atwork.
  • Employees may increase/decrease their contributions at any time by calling Fidelity at  1(800)343-0860 or logging onto www.Fidelity.com/atwork.
  • After 12 months of employment, working 1,000 paid hours and at least 21 years of age, Saint Francis Health System will match 50 percent of first 8 percent that the employee contributes

Paid Time Off (PTO)

  • Those eligible: full-time and part-time employees
  • Start accruing immediately upon employment
  • Paid time off for vacations, personal use, holidays and the first 24 consecutive hours of personal illness or injury.
  • May carry over unused PTO hours to PTO bank.  Maximum limit for PTO bank is 40 days (320 hours).
  • Full-time employees may accrue up to 21 days (168 hours) during their first through fifth year of service; 26 days (208 hours) during their sixth through 10th year of service; 31 days (248 hours) during their 11th year or more of service.
  • Part-time employees may accrue up to 10 days (80 hours) during their first through fifth year of service; 15 days (120 hours) during their sixth through 10th year of service; 20 days (160 hours) during their 11th year or more of service.
Saint Francis Health System recognized Holidays
  • New Year’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. 
  • Part-time and ECB employees receive an extra hour of base pay up to a maximum of 8 hours worked on a holiday.
Extended Sick Leave (ESL)
  • Those eligible: full-time employees
  • Start accruing immediately upon employment
  • Paid time off for personal illness or injury lasting more than 24 consecutive scheduled hours.
  • May accrue up to 7 days (56 hours) per year and carry over unused hours for a total of 1,040 banked hours.

Family and Medical Leave of Absence

  • Those eligible: full,  part-time and ECB employees
  • Eligible after 12 months of employment and 1,250 hours worked prior to leave.
  • Up to 12 weeks leave with retention of benefits and job following a temporary absence required due to birth or placement of a child or serious medical condition of employee, spouse, child or parent. (In accordance with policy.)
  • Leaves are subject to HR approval by following DOL guidelines.

Military Leave of Absence (LOA)

  • All employees who are engaged in the uniform services
  • Leave approval in accordance with USERRA Guidelines

Medical Leave of Absence

  • Those eligible: full-time and part-time employees whom are not eligible for FMLA or have exhausted their FMLA based on leave type
  • Leaves are subject to HR approval.

Tuition Assistance Plan

  • Those eligible: full-time and part-time employees
  • After completion of 6 months of continuous employment in an eligible status.
  • May receive reimbursement after class completion and necessary documentation is provided.
  • Full-time: $3,000/yr Basic, $4,500/yr Critical (per policy guidelines)
  • Part-time: $1,500/yr Basic, $3,000/yr Critical (per policy guidelines)
  • Requires employee to remain in a full-time or part-time working status for 6 months following completion of the course.
  • Grades and tuition assistance reimbursement form must be received no later than 60 days following the course end date to be eligible for reimbursement.
Health Zone
  • Those eligible: full-time, part-time and ECB employees
  • Immediate upon employment
  • Employee discount for Health Zone membership.
Ave Maria House 
  • Those eligible: full-time, part-time and ECB employees
  • Immediate upon employment
  • The Child Development Center is open Monday through Friday 6:20 AM to 8:00 PM and is located on the Saint Francis Hospital campus
  • The School Age Program for the Four Year Old program and the Before/After school program operates Monday through Friday 6:00 AM to 6:00 PM and is located at 5710 E 61st Street South.
Service Awards
  • Those eligible: full-time, part-time and ECB employees
  • Five years of continuous employment required
  • Service award gift and special recognition honoring employee's 5th, 10th, 15th, 20th etc. anniversary.

Jury Duty Policy

  • Those eligible: full-time and part-time Immediate upon employment
  • Time paid is not deducted from PTO, see policy listed on HealthNet for further specifics.

Bereavement Leave/Funeral Pay:

  • Those eligible: full-time employees
  • Immediate upon employment
  • Up to three days leave/24 hours of funeral pay for policy defined family members

 Adoption Benefits

  • Those eligible: full-time and part-time employees, one year of continuous service from most recent hire date
  • FT - $4,000 per adoption (max $8,000)
  • PT - $2,000 per adoption (max $4,000)
  • If employee and spouse are both employed by Saint Francis Health system, eligible amounts are allowed to one employee per family.
Credit Union
  • Those eligible: full-time, part-time and ECB employees
  • Immediate upon employment
  • A full service financial institution providing savings, investment and lending services.
Cafeteria
