TaxSaver Plan

Allow medical and dental premiums to be deducted Pre-Tax
-
Medical Spending Account – Ability to pay out-of-pocket health/dental expenses (expenses must be eligible under section 125 guidelines) that are not covered by medical, dental or vision plans with pre-tax dollars.

Daycare Spending Account – Ability to pay section 125 eligible daycare expenses that are incurred while working with pre-taxed dollars.

Election amounts for each of the accounts are a minimum of $260 and a maximum of $5,000 per year.

Deductions are withheld in equal installments throughout the year.

Reimbursements received are non-taxable.

This is a use-it-or-lose it program.

Contribution amount must be re-elected each year during Open Enrollment for the following calendar year.
Life Insurance

Long Term Disability Insurance

Those eligible: Exempt - upon full-time employment, Hourly - upon first of month following two consecutive years of full-time employment, McAlester employees excluded.

60% of base monthly earnings (not to exceed $10,000) payable to age 65 if employee continues to meet disabled definition according to plan.

Elimination period (period of disability before benefits start ): Exempt - 90 days, Hourly - 180 days.

Paid Time Off (PTO)
-
Those eligible: full-time and part-time employees

Start accruing immediately upon employment

Paid time off for vacations, personal use, holidays and the first 24 consecutive hours of personal illness or injury.

May carry over unused PTO hours to PTO bank. Maximum limit for PTO bank is 40 days (320 hours).

Full-time employees may accrue up to 21 days (168 hours) during their first through fifth year of service; 26 days (208 hours) during their sixth through 10th year of service; 31 days (248 hours) during their 11th year or more of service.

Part-time employees may accrue up to 10 days (80 hours) during their first through fifth year of service; 15 days (120 hours) during their sixth through 10th year of service; 20 days (160 hours) during their 11th year or more of service.
Saint Francis Health System recognized Holidays
-
New Year’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.

Part-time and ECB employees receive an extra hour of base pay up to a maximum of 8 hours worked on a holiday.
Extended Sick Leave (ESL)

Those eligible: full-time employees

Start accruing immediately upon employment

Paid time off for personal illness or injury lasting more than 24 consecutive scheduled hours.

May accrue up to 7 days (56 hours) per year and carry over unused hours for a total of 1,040 banked hours.
Family and Medical Leave of Absence

Those eligible: full, part-time and ECB employees
-
Eligible after 12 months of employment and 1,250 hours worked prior to leave.
-
Up to 12 weeks leave with retention of benefits and job following a temporary absence required due to birth or placement of a child or serious medical condition of employee, spouse, child or parent. (In accordance with policy.)

Leaves are subject to HR approval by following DOL guidelines.
Military Leave of Absence (LOA)

Medical Leave of Absence

Tuition Assistance Plan

May receive reimbursement after class completion and necessary documentation is provided.
