MyChart is your online portal to scheduling, prescriptions, test results, billing and direct messaging your provider's office.

Activate your MyChart account

Once you have activated your MyChart account, you can request your medical records from Saint Francis and Warren Clinic and upload them to your MyChart. Please have your photo ID ready, the process should take less than 10 minutes.
