Effective May 1, 2026, all Warren Clinic locations will no longer accept cash or check payments. Patients may continue to make payments online in MyChart, with credit/debit cards in the clinic, or by mailing payments to: PO Box 258898, Oklahoma City, Ok 73125-8898
Financial Assistance
Should substantial medical services be required and the patient is unable to pay the full amount, a payment plan will be arranged. For those who do not have the funds to pay, financial assistance may be available. Please review and complete the following application for consideration.
