Information Needed:
- Clarification on what outside records are received, including previous images and medical records that indicate the presence of a mass, nodule, or lesion.
- Required images and reports for any CT, X-ray, or PET scans.

Information needed for patients being referred to the Saint Francis Cancer Center:
Oncology:
- Most recent note from the referring provider
- Recent labs
- Any scans, biopsies, pathology, genetics documentation
- Demographics and insurance
Hematology:
- Same as oncology, plus a trend of labs to support the referring diagnosis.

- Biopsy and operative notes
- Pathology reports
- Imaging (CT, MRI, and/or PET scans)
